One procedure removes excess glandular tissue and the other removes excess fat. The type of procedure performed depends on the cause of the condition. In some cases, both are used.

If excess glandular tissue in the breast area is the cause of gynecomastia, then it is cut out from the breast tissue. Incisions are made in inconspicuous areas such as the areola or under the arm. The surgeon removes the excess glandular tissue along with excess fat and extra skin around the breast area.

If excess fatty tissue is the cause of the condition, the procedure will likely involve a liposuction of the fat tissue. A small incision would be made near the areola area and a small tube would be inserted to vacuum the fatty tissue out. Excess skin may be removed in order to ensure it sits firmly atop the breast.

Patients are at a risk for infections, skin injury, bleeding and allergic reactions to anesthesia. Moreover, the procedure may result in visible scars and noticeable changes in the breast area. If the breasts or nipples are not symmetrical after the surgery, a second surgery may be completed to correct the mistake.
